PQ-16 – Prodromal Questionnaire

Description 

The PQ-16 is a 16-item questionnaire which screens for symptoms of psychosis. 

Special Instructions 

Once the patient has submitted it, you may preview it on the uninitialed documents tab in the EHR. However, do not initial it or it won't later be pulled into your Mobile Note. 

Scores Produced 

PQ-16 Total (0-16). Each item marked “True” contributes 1 point to the PQ-16 Total.  

Interpretation 

Scores greater than or equal to 6 are associated with higher risk of psychosis
