CORE-10 – Clinical Outcomes in Routine Evaluation 10

Description 

The CORE-10 is a 10-item questionnaire used to screen for and monitor common areas of psychological distress, including anxiety, depression, trauma, physical problems, functioning, and risk to self. 

Special Instructions 

Once the patient has submitted it, you may preview it on the Uninitialed documents tab in the EHR. However, do not initial it or it won't later be pulled into your Mobile Note. 

Scores Produced 

  • CORE-10 Total Score (0-40) 
  • CORE-10 Mean Score (0-4) 

Interpretation 

Total Score: 

  • 10 and less – non-clinical range 
  • 11 to 14 – mild psychological distress 
  • 15 to 19 – moderate psychological distress 
  • 20 to 24 – moderate-to-severe psychological distress 
  • 25 or above – severe psychological distress
