NIDA-Modified ASSIST - NIDA-Modified Alcohol, Smoking, and Substance Involvement Screening Test

Description:

The NIDA-Modified ASSIST was adapted from the Alcohol, Smoking and Substance Involvement Screening Test (ASSIST), developed, validated, and published by the World Health Organization (WHO) as an effective screening tool for identifying substance use. It generates a substance involvement score that suggests the level of intervention needed.

Special Instructions

Once the patient has submitted it, you may preview it on the uninitialed documents tab in the EHR. However, do not initial it or it won't later be pulled into your Mobile Note.

  • Cocaine SI Score (0-39)
  • Hallucinogens SI Score (0-39)

  • Prescription Opioids SI Score (0-39)
  • Prescription Stimulants SI Score (0-39)
  • Sedatives/Sleeping Pills SI Score (0-39)
  • Street Opioids SI Score (0-39)

Interpretation

Level of risk associated with different Substance Involvement (SI) score ranges for illicit or nonmedical prescription drug use:

0-3 Lower Risk

4-26 Moderate Risk

27+ High Risk
