Ohio Scales for Youth - Youth - Short Form

Description: A 48-item self-report instrument measuring problem severity during the past 30 days, functioning, hopefulness, and satisfaction with behavioral health services.

Special Instructions: The child self-rates their problem severity during the past 30 days, functioning, hopefulness, and satisfaction scales.
 
Once the patient has submitted it, you may preview it on the uninitialed documents tab in the EHR. However, do not initial it or it won't later be pulled into your Mobile Note.

  • Problem Severity
  • Functioning
  • Hopefulness
  • Satisfaction

Interpretation

  • Problem Severity: Higher scores indicate more frequent problems
  • Functioning: Lower scores indicate more impairment
  • Hopefulness: Lower scores indicate more hope or wellness
  • Satisfaction: Lower scores indicate more satisfaction
