Care Teams Overview

Patients have complex needs and may be treated by multiple providers simultaneously. With Valant’s Care Teams feature, practices can effortlessly navigate collaborative scenarios and manage the diverse treatment and billing requirements for patients with multiple providers.

Care Teams Features

Use of care teams allows the ability to manage multiple assigned providers treating the same patient (e.g., a Psychiatrist seeing a patient for one diagnosis and a Therapist seeing the same patient for a different diagnosis).  

  • Provider Tracking: the "Care Team" tab in Persons & Institutions | Patients, will allow for tracking of multiple "Assigned Providers" and "Outside Providers" involved in a patient's treatment. “Outside Providers” includes Referral Sources and Primary Care Providers.   • Diagnoses Page: the "Diagnosis" page in the patient chart includes separate tabs for "All Diagnoses" and "Billing Diagnoses." In the "Billing Diagnoses" tab, provider-specific billing diagnoses lists can be created alongside the default list. Review the Diagnosis Management article for all of the functionality in this page.
  • Optimized Billing: when generating charges, the rendering provider will determine the set of billing diagnoses used, eliminating the need for manual adjustments. See the Diagnosis Management article for details.
  • Increased Appointment Availability Options: when a patient uses the Appointment Request tool in MYIO, they can search the availability of any of their assigned providers. They can also pick which facilities to search or leave the facility field blank to search across all facilities where the selected provider(s) have availability. 
  • Two columns can be added to Data Exports for showing all Assigned Providers and Outside Providers on a patient's Care Team. When multiple values exist in these columns, they will be separated by commas. These columns are added via Reports | Data Export Center Admin under the "Patient Providers and Guardians" section.
