Adding a fax number to a lab request

To include a fax number on the lab request slip, please do the following:

1. Click on Persons and Institutions | Providers
2.  Select a provider
3. Click the Preferences tab
4. Uncheck the "Use Practice Address for Lab Requests" checkbox.
